Informed Consent for Aesthetic & Injectable Treatment

For medical and injectable services (e.g., Botox/Dysport, dermal fillers, B12, IV therapy), the following informed consent applies in addition to the Release of Liability.
Nature of treatment
Neuromodulators (Botox/Dysport) are injected to temporarily relax muscles and soften lines; results develop over ~3–7 days and last roughly 3–4 months. Dermal fillers restore volume and contour and last several months to over a year depending on product and area. Re-treatment is required to maintain results.
Risks & side effects
Common, usually temporary: redness, swelling, bruising, tenderness, headache, or asymmetry. Less common: lumps, infection, allergic reaction, ptosis (drooping), or vascular events with fillers. I have had the opportunity to ask questions and discuss alternatives, including no treatment.
Contraindications I confirm I have disclosed
I certify that I am not pregnant or nursing; that I have disclosed any neurological conditions (e.g., myasthenia gravis, multiple sclerosis); known allergies (including to albumin or lidocaine); use of blood thinners; and any recent treatments in the area.
Consent
I consent to treatment administered by my licensed Habit provider in the areas determined appropriate during consultation. I understand I may decline or stop at any time.
